We are seeking a highly skilled Discharge Support Professional to join our team at Imperial College Healthcare. As a key member of our multidisciplinary team, you will play a crucial role in supporting patients throughout their care pathway.

Main Duties
  • To ensure quality care and support services that promote independence, dignity, and safety for patients.
  • To work closely with qualified staff and allied healthcare professionals to assess, plan, and implement patient care plans without direct supervision.
  • To provide in-reach support to local hospitals to facilitate earlier discharges, reducing lengths of stay and promoting timely transfers.
