NWPH Discharge Hub Clinical Screener ( Physio/OT/Nurse/Social Worker)
An exciting opportunity has arisen for a Physio/OT, Nurse or Social worker who would embrace a new challenge. Northwick Park Hospital Discharge Hub are looking for a Discharge Hub Clinical Screener within the Harrow Integrated Care Coordination Service (ICCS).
The successful post holder will be based at Northwick Park Hospital and work in partnership with colleagues in the acute sector, social services, continuing care and other community health services to ensure that appropriate patients from the acute setting are assessed and transferred safely to appropriate community-based services in a timely and seamless manner.
Main duties are assessing and deciding on appropriate discharge pathways for our most complex and challenging patients. This will include triaging referrals coming into the hub, liaising with external and internal partners, minimising delays in discharge and streamlining discharge process. The post holder will work proactively with the ward MDTs and across the IDT to identify and assess patients on the wards to facilitate timely discharge as an In-reach service. This will include acting as a trusted assessor with our intermediate care units.
Operating within the framework of the Hospital Discharge Services guidance (Aug 2020), the post holder will work in partnership with colleagues in the acute sector, Social Services, continuing care and other community health services to ensure that appropriate patients from the acute setting are assessed and transferred safely to appropriate community-based services in a timely and seamless manner.

Where necessary, to act as a the CLCH on-site Team Leader and to be responsible for day to day management of the community health based elements of service delivery; including acting as a line manager for any other CLCH staff within the team
