We are thrilled to present a truly pioneering opportunity for an experienced, driven, and talented pharmacist to advance their clinical expertise and contribute to the development of pharmacy services at Morriston Hospital's renowned tertiary cardiac centre.

This advanced pharmacist position, funded by NICE, will be directly embedded within the cardiology multidisciplinary team (MDT). This role is designed to enhance patient safety, optimise medication regimens, facilitate effective discharges, and ensure financial stewardship of cardiology prescribing.

Main duties of the job

Key Responsibilities

  • Lead, develop, and deliver patient-centred pharmacy services across cardiology alongside utilising prescribing skill set.
  • Collaborate with clinical colleagues in a fully integrated MDT to meet the pharmaceutical needs of the cardiac unit.
  • Participate in advanced practice opportunities and contribute to the ongoing development of cardiology services.
  • Create and present regular reports to the NICE high cost group with outcomes and improvements

The role will involve working collaboratively with pharmacy colleagues, clinicians, nursing staff and other members of the multi-disciplinary team to ensure the delivery of high-quality pharmaceutical care to cardiac patients. The role will be based within cardiology's MDT with a focus on the weekly ward take and facilitating homecare prescriptions.

The role will also involve identifying areas for improvement in pharmaceutical services within the cardiac unit and implement strategies to enhance efficiency, quality, and patient satisfaction.

Supervision, mentoring and training of junior colleagues will also be a major component of the job.

Delivering an independent prescribing service and managing pharmacy aspects of the Cardiac Home Delivery Service.
