The Community Complex Care Team's aim is "to empower and enable patients with complex needs to reach their full potential by providing compassionate and caring family-centred care". This unique service provides complex care packages for patients (adults and children) with continuing complex health care needs across North and South Staffordshire and Shropshire.

An exciting opportunity has been developed to join our Specialist Bank Service. This service has been designed to meet the needs of patients with complex needs with gaps in their care provision due to staff sickness or short term vacancy. Support and training can be offered to ensure all staff are confident and competent caring for patients with complex needs. Midlands Partnership NHS Foundation Trust is an excellent place to work. Minimum qualification and experience: Registered Nurse (child or adult branch). Acute or community experience required.



Main duties of the job
  • To provide high standards of care to patients with highly complex continuing care needs. Care will be delivered within the patient's own home / other community setting or local hospital. The post holder will be required to deliver care independently but will be expected to report back to other members of the team, the patient and family. The post will involve lone working; there is always a qualified nurse on-call for support.
  • Assist in the development of new patient care packages, which may include discharge planning, in conjunction with the Team Leader
  • Assess patient care needs; develop, implement and evaluate programmes of care with the patient and their family. Adhere to legislation, ensuring a high standard of care and continuity.
  • Produce contingency plans required within the package. Ensure that all staff within the team are aware of these plans, practice and procedures as they relate to the care of the patient and their equipment.
  • Complete and evaluate risk assessments for patients and staff as required with regular review as new information is received or changes occur.
  • Undertake and be responsible for the practical teaching of unregistered staff with completion of relevant documentation, identifying relevant teaching packages and patient specific skills frameworks. Able to organise own time and that of trainee staff to ensure this is done in a timely manner.

Detailed job description and main responsibilities

  • Accountable for ensuring all unregistered staff working alone with their allocated patients have completed necessary patient specific skills training. Ensure these are reviewed at planned intervals (at least 6 monthly) and as and when required, working alongside staff regularly to ensure compliance and to further build on their knowledge and skills.
  • Lead and manage designated staff, ensuring completion of regular clinical and managerial supervision, appraisal, support, staff development and training, ensuring documentation is clear and robust, in line with Trust policy.
  • Ensure that any complaints are directed to PALS and the Team Leader made aware, according to Trust policy.
  • Attend multidisciplinary review meetings to provide input from our team on the package of care and be aware of any arising issues
  • Be knowledgeable and adhere to the NMC Code
  • To promote safeguarding in line with Trust policies and practices relating to child protection and Vulnerable Adult issues.
  • Ensure self and others adhere to the Trust's Medicines Management policy and Pharmaceutical Code of Practice. Reporting any incidents actual/potential that occur according to Trust policy.
  • Ensure that accurate and up to date client records are kept according to legislation and confidentiality.
  • Attend a variety of team meetings as required.
  • Maintain up to date knowledge of Trust Policies and Procedures and disseminate these to own staff.
  • Use material resources with regard to efficiency and effectiveness to avoid waste.
  • Take responsibility for ensuring adequate stock for designated patients if indicated. Ensures equipment stock levels are maintained in conjunction with the administrative team.
  • Promote Trust values and acts as a role model.
  • Identify own training needs and objectives by regular Performance and Development Reviews with the Clinical Nurse Specialist.
  • Keep up to date with all professional developments. Attend all study days and courses identified and required including mandatory updates.
