Triage Nurse

We are seeking a highly skilled and enthusiastic Band 5 NMC Registered Nurse to join our Out of Hours (OOH) Service as a Triage Staff Nurse. This is a part-time position, minimum 15 hours per week, where you will be responsible for remote triage of calls and referrals made to the North and South Staffordshire OOH community nursing teams.
Main Responsibilities- Make appropriate decisions following comprehensive remote triage to ensure the safe management and allocation of visits where needed, in response to calls and referrals received by the community nursing service.
- Determine eligibility of patients referred to the service and collect additional information to support decision making. Transfer or redirect referrals to appropriate internal or external services as necessary.
- Liaise with patients, families, carers, and other service providers to support clinical decision making and identify appropriate management of referrals and messages.
- Liaise with the nurse in charge of the appropriate nursing team to allocate urgent visits and discuss potentially complex referrals.
- Review all referrals and messages to the community nursing service, including an understanding of symptom management of deteriorating patients, palliative and end of life care, wound care, bladder catheter and bowel care, recognition of deteriorating patients and potentially life-threatening conditions, and complex patients with co-morbidities.

To be successful in this role, you will need:
- Experience of working as a Community Nurse
- NMC Registered
- Further post-graduate training (desirable)
