Sunderland Ward is a 40 bedded community rehabilitation ward providing care to patients with often complex and challenging health and social care needs and we are seeking dedicated and motivated Assistant Practitioners who possess good communication skills and the ability to demonstrate assessment and problem solving skills alongside a passion for patient centred care.


You must have achieved Module 1 and Module 2 of the Level 4, Certificate in Healthcare (60 credits at level 4) and be committed to successfully completing modules 3 and 4 of Level 4, Certificate in Healthcare (60 credits at level 4), within one year of appointment.


You will be working alongside and with the supervision and support of registered nurses and be responsible for the management of patients, participating in the provision of holistic assessments and care management from admission to discharge.

Working closely with the multi-disciplinary team which is made up of physiotherapists, occupational therapists, speech and language therapists and health care support workers, you will be supported to work autonomously within your scope of practice and gain skills in a variety of clinical areas including cognitive impairment/dementia, frailty, palliative and end of life care needs, complex wound care management and rehabilitation.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
