About the Role

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at the Coastal Health District. As a key member of our public health team, you will play a vital role in promoting healthy lifestyles and preventing disease in diverse communities statewide.

Key Responsibilities
  • Assess Client Health Needs: Conduct comprehensive assessments of client health care needs, identifying areas for improvement and developing personalized care plans.
  • Provide Nursing Care: Administer nursing care to Health Department clients, including treatments, medications, and preventative/maintenance care.
  • Consult and Coordinate with Healthcare Team: Collaborate with healthcare team members to assess, plan, implement, and evaluate client care plans.
  • Monitor Client Care: Continuously monitor all aspects of client care, including diet and physical activity.
  • Provide Education and Support: Educate clients, families, and community service providers on health care, first aid, and immunizations.
  • Provide School-Related Health Care Services: Provide health care services to students in schools, including first aid and immunizations.
Requirements
  • Education: Bachelor's degree in Nursing from an accredited college or university.
  • Licensure: Current Georgia license to practice as a Registered Professional Nurse.
  • Certification: CPR certification.
  • Experience: One year of PHN experience or completion of a PHN course within one year of hire.
