Healthcare Assistant Practitioner

About the Role
We are seeking a skilled and dedicated Healthcare Assistant Practitioner to join our team at Hywel Dda University Health Board. As a key member of our Community Nursing Team, you will play a vital role in providing high-quality care to patients in their own homes.
Key Responsibilities
- Deliver high standards of care to patients and their families/carers within the home or clinic setting.
- Provide wound care/management, administration of medication, venepuncture, catheterisation, and continence promotion.
- Conduct holistic re-assessments, record observations, and promote health.
- Liaise with staff, patient/carers, and other agencies/organisations to ensure effective delegation of duties and risk management.
Requirements
- Level 4 vocational qualification in a relevant healthcare-associated subject/Certificate in Healthcare Support Worker Education.
- GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
- Evidence of continuing personal development relevant to the role in healthcare.
- Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines, and the All Wales Band 4 Assistant Practitioner Governance Framework.
- Clinical knowledge relevant to the role.
Desirable Qualifications and Experience
- Local knowledge of statutory and voluntary services.
- Teaching or assessment qualification.
- Experience in a range of clinical skills, including palliative care.
- Administration of medicines via oral, topical, SC, IM routes.
- Skills in non-complex wound care.
