We are looking for an enthusiastic and dynamic Dietitian to join our well respected community dietetic team in Pembrokeshire to provide a specialised clinical dietetic service for the acute / urgent response team. We welcome applicants that would like to consider flexible and part time working (minimum 4 days per week).

This is an exciting post and offers an ideal opportunity for a Dietitian to fully embed themselves as part of a wider MDT, supporting embedding nutrition/hydration as part of models of care. The post-holder will be based in South Pembrokeshire hospital alongside the integrated care team.

Acting clinical dietetic lead for the rapid response team / community integrated care team, working collaboratively with proximal community multidisciplinary services to strengthen communication and clinical delivery across community settings.

Autonomously manages a varied and complex caseload of adult dietetic referrals with nutritional support needs working as part of the community team and wider multi-professional team to provide specialist dietetic advice in relation to nutritional management and to produce a coordinated care plan of care.

Leads on embedding and strengthening nutritional screening at the ‘front door’ and as part of rapid response pathways and models of care including providing training and education, including accredited training to therapy assistant practitioners and others working as part of the front door model to de-escalate risk

Supports the wider community nutritional support team by providing operational leadership alongside other community leads, ensuring effective prioritisation of referrals and that dietetic pathways and processes are fully embedded.

Delivers dietetic care in a variety of settings to meet the needs of the patient including undertaking home visits, attending care home/residential settings, community hospitals and providing group education and working across virtual platforms.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. **Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
