About the Role

We are seeking a highly skilled and dedicated Assistant Practitioner to join our team in the Pembroke District Nursing Team. As an Assistant Practitioner, you will play a vital role in providing high-quality care to patients in their own homes, working closely with experienced District Nurses and other healthcare professionals.

Key Responsibilities
  • Deliver high standards of care to patients and their families/carers, including wound care, administration of medication, and venepuncture.
  • Work collaboratively with staff, patients, and other agencies to ensure effective delegation of duties and risk management.
  • Provide holistic re-assessment, recording of observations, and health promotion to patients.
  • Recognize and escalate care for unwell patients, ensuring effective communication between those involved in the care package.

Requirements
  • A Level 4 vocational qualification in a relevant healthcare subject or equivalent.
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications.
  • Experience of providing relevant clinical care, with a focus on wound care, medication administration, and venepuncture.
  • Excellent interpersonal and communication skills, with the ability to manage and diffuse challenging situations.
