About the Role

Hywel Dda University Health Board is seeking a skilled Community Mental Health Practitioner to join their team at the Haven Way Day Hospital in Pembroke Dock, Pembrokeshire. Main Responsibilities
  • Assess care needs and develop programmes of care for a defined clinical caseload, working in collaboration with healthcare professionals and statutory agencies.
  • Implement and evaluate programmes of care, ensuring effective communication and positive working relationships with all stakeholders.
  • Work independently with a delegated caseload, utilizing effective communication skills and developing positive working relationships between healthcare professionals, statutory and voluntary agencies.

Person Specification
  • Professional Qualification/Registration from a recognised body (e.g., RMN, Occupational Therapist, or Social Worker).
  • Post-basic education in the area of practice to postgraduate diploma level.
  • Post-registration experience.
