At Lewisham & Greenwich (LGT) we are pleased to announce, that in conjunction with Roald Dahl's Marvellous Children's Charity we are seeking to recruit a highly motivated and enthusiastic Children's Nurse to support in our Transition programme.


University Hospital Lewisham & Queen Elizabeth Hospital Woolwich, are very busy district hospitals, providing a wide range of hospital and community services to hugely diverse populations with large numbers of families living in poverty and challenging complex family structures across South East London and beyond.

The Trust provides Paediatric services across several boroughs which include; Lewisham, Bexley, Greenwich and within our local community services.

In addition, the Trust transitions young people from other tertiary specialist providers to our adult services e.g. from Evelina Children's Hospital and Great Ormond Street.


Approximately 35% of teenagers admitted to our services have a chronic health condition, and many young people accessing our services have complex needs.

LGT provides local, specialist and community care for children and young people with a range of chronic and complex conditions.


The successful post holder will work along side the current Transition Coordinator, supporting young people with long term conditions as they transition from Children's to Adult services.


They will support specialist professionals in both paediatric and adult services, helping them structure resources to meet the complex needs of these young people.


This post will have a focus on ensuring young people accessing health care at LGT were appropriately sign posted to other organisations and that the Trust begins to look at young people's health and respond appropriately.


The post holder will also work with community teams to ensure that young people with complex health needs and those with ASD etc are supported appropriately and the hand over is fluid and effective.


In partnership with Roald Dahl's Marvellous Children's Charity, we are looking for a motivated and dynamic nurse with experience caring for young people with chronic health conditions.

Lewisham and Greenwich NHS Trust is a community focused provider of local and acute care, providing high quality services to over one million people living across the London boroughs of Lewisham, Greenwich and Bexley.

With a turnover of around £700 million, we provide services at Queen Elizabeth Hospital in Woolwich, University Hospital Lewisham and a range of community settings in Lewisham.

We also provide some services at Queen Mary's Hospital in Sidcup.

This is reflected in our most recent Care Quality Commission (CQC) inspection (from February 2020), which found improvements across the organisation and gave us an improved rating of "Good" in the Well-Led and Effective domains, in addition to the rating of "Good" that we had already achieved in the Caring domain.

Our staff culture change programme has been key to the Trust's improvement journey, as we've stabilised our workforce by reducing vacancy rates and improving staff retention.

We've also significantly increased the representation of minority ethnic leaders across our clinical and corporate teams.

Job Summary:

  • Aim of the role


A key part of the Children's and Young people's transition strategy is developing a model of care for Young people accessing services across Lewisham, Greenwich and Bexley.

The post holder will work along side the Lead Transition Co-ordinator, working with the Children's Health leadership teams on both sites.


This will include:

  • Providing professional and clinical support for the care of Young people wherever that are cared for in the Trust
  • Work closely with the multidisciplinary team to provide support, advice and education for teenagers and young adults in inpatient and outpatient setting
  • The post holder will not hold a case load but will support both Children's and Adult Clinical Nurse Specialists with Transition planning and adolescent lifestyle
  • To act as a resource for teaching and educating patients and their families
  • Support with the running of LGT Youthboard, to ensure the views of Young people are intrinsic in the models of care developed
  • Supporting with the expansion of the current Transition programmes, allow for additional service cover.
