Job Summary:

Care Coordinators are essential in a PCN, coordinating care and support for people with long-term conditions and the elderly.

They work at our SPA and rotate through services like e-Hub, Secondary Care Referrals, Community Dermatology, and Enhanced Cardiology Services.

We provide exceptional care to all patients and are always introducing innovative services like the Virtual Group Consultation for diabetic patients.

This role is vital in delivering these services.

Key Responsibilities:

  • Acquire and sustain knowledge of service pathways and criteria.
  • Keep electronic records of all referrals and interventions to evaluate the impact of the service and enhance other local services.
  • Provide administrative assistance for incoming eConsults, manage relevant adminrelated eConsults, and redirect others to the appropriate team.
  • Ensure sufficient phone coverage is available for all services, respond to any queries professionally, and provide helpful information.
  • Book appointments for suitable referrals, make changes or alterations as required, and support patients in booking their secondary care appointments online.
  • Keep accurate records of all activities, maintain templates for onward referrals to secondary care services, and deal with queries sensitively and in a timely manner.
  • Monitor and complete tasks within the SystmOne system, distribute incoming mail or faxes as appropriate, and workflow eConsults into patient records on SystmOne.
  • Maintain effective communication across the team, actively participate in the development of the eHub, and perform other administrative tasks as necessary.
  • Provide administrative support to Central London Healthcare and attend their events and activities in consultation with their line manager.

Qualifications:

  • Essential: GCSE grade A to C in English and Maths.

Knowledge and Skills:

  • Essential: Computer literacy and excellent communication skills (verbal and written).
  • Desirable: Good knowledge of primary care and local services, personalisation and public health agendas.
  • Numeracy and literacy skills are also essential.

Personal Attributes:

  • Essential: Willingness to learn and work in a team or autonomously, solutionfocused, tactful and diplomatic, able to manage a high level of tasks and information, and emotionally resilient.
  • Experience: At least 1 year of working in a customer service orientated environment, and desirable experience in Primary Care with excellent SystmOne skills.

Salary:
£26,000.00 per year
