See main duties and full JD The main duties and responsibilities under primary health care facilitation are to: be the key contact for cancer screening and health facilitation in Camden/Islington promote health facilitation to individuals and groups in Camden/Islington and encourage a consistent approach within NCL contribute pro-actively to the work of the Camden Learning Disability Partnership Board (Planning Together) and also its equivalent, Islington Partnership Board, as well as the Health and Wellbeing Sub-Group in order to enable improvements in health facilitation, to report progress and to share information about implementing improvements; provide expert advice based on evidence-based practice and to keep up to date on the latest evidence for effectiveness; contribute to the continuing development of the joint strategic needs assessment by ensuring that needs and data and the views of people with learning disabilities are captured in the joint strategic needs assessment; ensure that standards identified in Valuing People and in National Service Frameworks and related policies are routinely applied in relation to people with a learning disability; people with LD to participate in screening, e.g.

attend or self-sample. Upskilling family/ carers and social care workers as sample takers.
