An exciting and unique opportunity has arisen for a motivated, enthusiastic individual to join the LGI Cancer Services team in a new substantive position of Patient Navigator LGI Cancer at London North West University Healthcare.


The Cancer Services team have implemented an early diagnostic pathway for patients with a suspected cancer to achieve definitive diagnosis by day 28 following referral by the GP.

The navigator will be involved from the beginning and throughout the pathway to work both directly and indirectly with patients, their carers and members of the wider MDT to assist in coordinating the patient pathway.

The role of the navigator will be to improve the timeliness of the patient journey and the patient's experience.


An ability to work autonomously and to respond quickly to requests is required in addition to the ability to use your own initiative.


  • The post holder is responsible for supporting patients and their families through the LGI cancer pathway across secondary, community and primary care. They will be responsible for coordinating care, education and support for people with complex care needs and providing a point of access to patients and carers.
  • The post provides dedicated administrative support for the Faster Diagnosis Standard (FDS) and Inter Trust Transfer on a suspected/confirmed cancer pathway. The role will facilitate the Trust in achieving and maintaining the FDS and overseeing the transfer of patients between referring Trusts for both MDT discussion and treatment.
  • This role has been specially designed to support existing pathways and will require the post holder to evolve to support the pathways as they are continuously improved over time. The post holder must be adaptable and willing to work flexibly as the service develops, which includes quality assurance, service development and improving patient outcomes and experience.
  • The post holder will work both directly and indirectly with patients and their carers to assist in coordinating the care through an accelerated pathway from referral to treatment of LGI cancer. The development of the role will be led by the needs of the patient. The role will also include providing the patient with the relevant information and coordinating any tests and appointments that are required to expedite the patient pathway.

KEY RESPONSIBILITIES

  • Ensure all patients on a 62 day cancer pathway (62 day standard, screening and upgrade) reach the Faster Diagnosis Standard by day 2
  • Proactively manage patient case load to aid the early detection of delays within the pathway escalating any bottle necks to the Cancer Pathway Managers and /or relevant service.
  • This post will evolve over time as the pathway begins to evolve and crystallise and the post holder will be required to adapt and undertake any other duties as may be required by the cancer services directorate or wider healthcare team that are consistent with the responsibilities of the grade
  • To comply with the Trusts and departments policies, guidelines and procedures.
  • Maintain complete confidentiality at all times. During the course of these duties, the post holder will have access to confidential and sensitive material about staff and the organisation and on no account must this information be divulged to anyone other than authorised persons as directed by the Senior Nursing Team.
Coordination of Care

  • Ensure patient appointments are booked in line with the timed pathway protocols
  • To be a point of contact for GPs who have queries regarding their patients that have been referred to the LGI service as an urgent 2WW
  • Liaise with the endoscopy, radiology and pathology departments to ensure patients are dated within the timelines as dictated by the relevant specialty.
  • Coordinate the necessary assessments, appointments or investigations as identified.
