Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services

Main Duties:

  • To complete care act assessments to determine eligibility
  • To complete care and support plans, where eligible needs are identified.
  • To ensure community resources are drawn upon and work is strength based and progressive in nature
  • To complete initial reviews/ annual reviews
  • To work with those under 18 to plans for their adult support at 18 (via assessments)
  • To work with those with Autism and no learning disability. Work may also include working with those with other neurodiversity's or vulnerabilities, not supported through other social care teams.
