The team is made up of a Crisis/Assessment/intervention team and a Treatment/Therapy team.

We have several pathways within the service offering intervention to children and young people up to the age of 18. Working closely with family, carers and the professional system to achieve the best possible outcomes and support healthy transitions.

Main duties of the job

Assess and manage risk usingevidence based risk assessment tool.

To formulate risk management plan with young person and carer/family, under supervision of care coordinator or clinical lead.

To provide comprehensive mental health assessment, formulation and collaborative care planning with young person and carer/family, under supervision of clinical supervisor or clinical lead.

To provide, under the direction of a Clinical supervisor or clinical lead effective treatment and intervention to young people and families with complex and substantial needs.

To deliver evidenced based clinical and psychologically based Interventions.

Work as a member of the multi-disciplinary team.

Work with partner agencies including schools, children's social care and third sector organisations.

Ability to work flexibly to meet the needs of Service Users across pathways which may regularly include working extended and flexible hours.

Demonstrate flexibility in the role. This may involve working across other clinical pathways with in CYPS services to support the needs of the service, whilst ensuring the delivery of high quality care at all times.

We work mainly in the community seeing children & young people and their families within clinic's, their homes and sometimes online via TEAMS. We also see young people in hospital if in Crisis.

Job description

Job responsibilities

Provide assessment, formulation, and intervention to children and young people with complex mental health needs & learning disabilities. Which may include group work.

To deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Clinical lead and clinical Supervisor.

Work with both children & families

To manage case work under direction of the Clinical Lead.

The post holder will work as a member of the multi-disciplinary team ensuring that high quality, individualised care is delivered and that this promotes Recovery and Wellbeing and maximises Independence. Working with Third sector organisations.

To provide compassionate care that is based on empathy, kindness, respect and dignity.

To incorporate a client centred philosophy and client centred collaborative care.

Ability to work flexibly to meet the needs of the young people and families across pathways, whilst always ensuring the delivery of high-quality care. Ability to work flexibly to meet the needs of Service Users and families across pathways which may include working extended and flexible hours over 7 days a week, if on the crisis pathway.

The role may involve working across all childrens pathways within childrens Specialist mental health & Learning disabilities service.

Education and Qualification

Essential

First Level Registration RMN/RNMH/RNLD Qualification Experience of working with service users and carers in successful delivery of treatment plans Complete preceptorship programme for newly qualified nurses Multi-disciplinary team work. Proven ability to develop therapeutic partnership with service users and carers. Proven ability to be able to communicate interventions to service users.

Experience of working with children and young people

Knowledge and Experience

Essential

Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions. Understanding of the statutory issues which impact on service users.
