About the Role

We are seeking a highly skilled and motivated Community Health Nurse to join our team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working under the crisis model and pathway.

Key Responsibilities
  • Work as part of a multi-disciplinary team to provide 24/7 crisis services as an alternative to hospital admission.
  • Assess and manage patients with a variety of mental health needs, including those with risk and complexity.
  • Develop and implement individualized care plans to support patients in their recovery.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Contribute to the development of the team and the service, identifying areas for improvement and implementing changes to enhance patient outcomes.
Requirements
  • Professional Nursing qualification (RNLD/RGN).
  • Continued professional development and a commitment to ongoing learning.
  • Experience of working with service users relevant to the post, including those with mental health needs.
  • 12 months post-registration experience and completion of preceptorship.
Desirable Qualifications and Experience
  • Therapy-based training.
  • ENB 998 / P126/127.
  • Student Assessor/Mentor.
  • Post-registration clinical experience.
Knowledge and Skills
  • Knowledge relevant to the post, including safeguarding, risk management, and health and safety legislation.
  • Ability to work collaboratively as part of a multi-disciplinary team.
  • Excellent communication and interpersonal skills.
  • Ability to meet deadlines and manage competing demands.
  • IT skills and ability to use patient information systems.
