About the Role:
We are seeking a highly motivated and enthusiastic Band 6 Community Psychiatric Nurse to join our Cumbria Community Mental Health Team. As a key member of our team, you will work closely with our multidisciplinary team to provide high-quality care to individuals with mental health needs.
Key Responsibilities:
- Provide comprehensive assessments and care planning for individuals with mental health needs
- Develop and implement individualized care plans in collaboration with the multidisciplinary team
- Work closely with community teams to ensure seamless care transitions and coordination
- Participate in quality improvement initiatives to enhance patient outcomes and experience
Requirements:
- Registered Mental Health Nurse with a valid NMC registration
- Experience working in community mental health settings
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
