Job summary

The post holder will work as part of a multi-disciplinary team and within their competencies to provide expertise in clinical medicines management and to develop and participate in delivery of the pharmacy service to the PCN, in line with the requirements of the NHS Long Term Plan and the national PCN Direct Enhanced Services.

The post holder will be required to undertake the national CPPE Primary Care Pharmacy Education Pathway and will be supported to grow into the role if previous experience is not present.

You will be part of a team of pharmacists and Pharmacy Technicians working to support safe, effective prescribinng for our 36'000 patients.

Main duties of the job

We are looking for a pharmacy technician who has a passion for patient care and wants to develop or build on existing skills in medicines management, supporting our 'prescribing hub'.

They also support the clinical pharmacists to deliver medication reviews, including supporting and assisting activities across the PCN. The intent is to maximise best practice including safe, cost effective prescribing to improve the quality of patient care. They will help patients to get the best from their medicines through timely medicines reconciliation on transfer of care, medication use reviews, identification of patients suitable for electronic repeat dispensing, providing assurance of monitoring of high risk drugs, and handling of prescription queries. The role will also include training and development of practice based prescribing clerks and administrators.

You will need passion and drive with an eye for detail. You will be a fantastic ambassador for the PCN showcasing the immense difference good pharmaceutical knowledge and skills can bring. It is important to us that you are eager to learn and we will support you on your journey to develop your skills within the PCN.

This role is pivotal to improving quality of pharmaceutical care and medicines related operational efficiencies across the PCN and requires motivation and passion to deliver an excellent service.

Support the role of the clinical pharmacists at Exeter City PCN in delivering medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings. Medicines reconciliation post discharge and on other transfers of care Carry out medicines optimisation tasks including effective medicine administration ( checking inhaler technique). Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively. Day to day handling of prescription queries within area of own competence Delivery of training and development to practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within General Practice Report on key performance indicators to demonstrate added value for patients and General Practice in accordance with NICE guidance Use and maintain information reporting systems and databases relevant to the position including incident reporting, medicines optimisation, prescribing data, cost savings, and patient outcomes Liaise with other pharmacy stakeholders Medicines Optimisation team, community pharmacy, and hospital pharmacy teams Handle function specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldicott Guidelines and the Confidentiality Code of Conduct Be responsible for the organisation and planning of own workload to meet set deadlines Utilise GP practice clinical systems to conduct clinical and prescribing audits EMIS Web, SystmOne Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result Carry out other duties which are appropriate to the skills and competencies of the post holder and grade of the post as the priorities of the service change Establish and maintain effective working relationships with GP practices, ICB staff and other healthcare professionals/stakeholders across the interface Promote cost-effective, safe, evidence based prescribing in accordance with national guidance and the local Medicines Optimisation Strategy Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients;
