Job summary

Provide visible, high calibre nursing care and support to HMP Wandsworth prison healthcare teams, within a multi-disciplinary framework for adults with learning disabilities, Autism and/or complex needs relating to neuro diversity.

The post holder is to conduct triages, clinical reviews and assessments to formulate person centered care plans and communication aid to assist those with learning needs and Autism. They are to maintain high standards of practice in line with local and trust policies whilst adhering to NICE guidelines.

To offer support to the multi- agency teams regarding those with learning needs and Autism, to attend ward rounds and liaise closely with the mental health teams for joint working and continuity of care.

Post holder will contribute to the development of good clinical practice promoting the clinical and professional development of staff to ensure they provide holistic and evidence-based care for those with learning needs and Autism.

The post holder will also provide deputy management duties within this unit and participate in the overall development of the Service.

Main duties of the job

Participate in the assessment and treatment of those with learning disabilities and autismentering the establishment and support other nurses to undertake this task.

Promote and develop communication systems which enable staff to work in a person-centered way with service users.

Ensure the provision of evidence based practices in managing challenging behaviors and reducing the health inequalities.

Actively participate in the adoption of a person-centred holistic approach to service user's assessment and care planning using contemporary models of care. To involve service users with autism and mild/moderate learning disabilities in these processes

Demonstrate evidence based guidance on person centred/holistic assessment, care planning, intervention and evaluation for adults with learning disabilities /autism

Ensure the principles of Valuing People and Transforming Care are embedded in practice along side NHS England guidelines on supporting offenders with learning disabilities and autism in prison.

Ensuring Safeguarding Vulnerable Adults policy is understood and adhered to and that issues are reported appropriately

Demonstrate understanding the importance of developing good practice in issues relating to challenging behaviours and people with neuro diverse conditions.

Job responsibilities

IMPORTANT INFORMATION, PLEASE READ:

All applicants must be willing to undertake National Security Vetting in order to work in a Prison Setting. This will be completed as part of the pre-employment checks through Oxleas and the prison vetting team.

Non-UK passport holders will need to have correct documentation (right to work in the UK) and a Home office Share code.

Address History:

5 years address history will be needed.

Applicants that are not UK Passport holders who provide less than 5 years UK address history will need to provide a Police Certificate which must be in English from where they resided previously.

Applicants who are UK Passport holders who have lived abroad for a period of more than six months during the last three years will need to provide a certificate of good conduct or an overseas police check in English from the countries resided in or visited .
