Job Summary

We are seeking a highly skilled and dedicated Learning Disability Nurse to join our team at Central London Community Health Trust. As a Community Learning Disabilities Nurse, you will play a vital role in providing specialist care to individuals with learning disabilities and complex physical health needs.

Main Responsibilities

  • Provide high-quality nursing care to individuals with learning disabilities and complex physical health needs in a community setting.
  • Work collaboratively with a multi-disciplinary team to coordinate care and support for individuals with learning disabilities.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct regular assessments and reviews to ensure the best possible outcomes for patients.
  • Participate in Care and Treatment Reviews to ensure seamless care coordination.

Person Specification

Essential

  • Qualified Learning Disability Nurse or Adult Nurse with experience of working with individuals with learning disabilities.
  • Post-qualification experience in learning disabilities nursing in a community environment.
  • Experience of working with a multi-disciplinary team and coordinating care for individuals with complex needs.

Desirable

  • Experience of Care and Treatment Reviews.
  • LeDeR trained reviewer.
