An exciting opportunity has arisen for a neighbourhood nursing night team leader to work with our night nursing team within Medway.

We have a permanent band 6 position for an enthusiastic and dedicated registered nurse with substantial previous experience in community nursing. This is an excellent opportunity to work in a supportive environment as a Team Leader, developing high level skills in patient assessment and care and contributing to leadership within the team.

The post holder will be expected to work flexibly across the neighbourhood nursing service, however, this post is focused on the night nursing service which currently works between 7pm and 7am, 365 days a year. The night team covers the whole of Medway and delivers essential nursing care to patients alongside other out of hours colleagues. Registered nurses work alongside healthcare assistants to deliver safe and effective care overnight. Occasional day time work will be required to meet your learning and development needs.

Main duties of the job

You will assist the clinical Lead in the co-ordination and delivery of care in line with local and national district nursing strategy. Your role will involve:

Assessment of new and complex patients Delivery of comprehensive senior nurse reviews of patients in their own homes and other community care settings Working as part of a wider multidisciplinary team Supporting new staff in learning and development in the service Development of the team through leadership and role modelling Management of staffing rotas and allocation of clinical work Establishing and maintaining effective communication with all clinical staff, patients and families

You will have the ability to work contracted hours rotationally across day and night shifts, as reasonably required by the needs of the service. The post holder must demonstrate professional leadership and be passionate about patient care. They will need to be able to problem solve and adapt to challenging situations, manage resources efficiently and ensure the delivery of safe, personal and effective patient care. We are looking for a nurse with a commitment to quality care and the ability to positively challenge to ensure patient and staff safety.

Evidence of further study

Mentorship or training qualification

District Nursing qualification / community practitioner pathway or willingness to commence within 1 year of starting

Relevant post-graduate qualification, in elderly care, chronic disease management, long term conditions, rehabilitation

Experience

Post graduate accumulative experience in the fields of elderly/ rehabilitation/ community/ intermediate care/ long term conditions/acute illness.

Experience of delivering education and teaching.

Proven and evidence of experience in an autonomous practitioner role

Experience of working in the community in healthcare.

Experience and competence in high-level clinical assessment of complex patients

Evidence of specialist knowledge in minor illness management, community nursing and advanced practitioner

Experience of using technology to facilitate provision of patient care

Experience of motivational interviewing

Change management experience

Special knowledge / expertise

Knowledge of current issues and trends within community healthcare, urgent Care, long term conditions, intermediate care and the NHS in general

A working knowledge of the admission avoidance ethos and facilitation of timely discharge

A sound knowledge of clinical governance and risk management

Ability to practice at an advanced level

Experience of lone working and decision making

Experience of mentoring students and other healthcare professionals

Proven ability to manage a caseload

Intermediate IT skills (Windows, Internet, Outlook, Excel, EPEX)
