**JOB TITLE**:Health Care Practitioner
**RESPONSIBLE TO**: Senior Health Care Practitioner/Nurse in Charge
**SUMMARY OF RESPONSIBILITIES**
1. May be full-time permanent post, but will require the undertaking of rotational duties including weekends.
2. Will assist the qualified nursing staff in the care, supervision and rehabilitation of residents.
3. Appropriate instruction in the detailed duties will be supervised for inexperienced staff by the Care Standards Lead or through separate arrangements made by the Manager.
**DETAILED DESCRIPTION OF DUTIES**
1. Acting as a Key worker for named residents.
2. Reception of residents - assisting with resident admission duties to include care of clothing and property.
3. Providing the care and support as detailed in each individual resident’s plan of care within the confines of your training.
4. Care of bed accessories and appliances, cot sides and ambi-lifts and reporting any faults to Nurse in charge.
5. Assisting residents with washing, bathing, changing clothes, hair and nail care, oral hygiene etc. as their care plan details.
6. Preparing residents for meals and assisting residents with their meals or fluids.
7. Ensuring clean and tidy environment throughout the Home for all residents.
8. Encouraging residents to maintain and develop increasing independence.
9. Making accurate. Detailed entries in the relevant care record for each resident. Reporting any concerns to the Nurse in Charge.
10. Use the resident’s individual plan to engage with each resident. Assist the Engagement Lead with following each residents plan.
11. Other general duties to help with the smooth running of the Home as specified by the Nurse in charge.
12. Reporting any changes and/or incidents to person in charge.
13. Have a good understanding of the S.O.V.A. reporting process.
14. Ensuring all personal mandatory training is up to date.
15. Attend all Supervision sessions as required.
**Job Types**: Full-time, Part-time, Permanent
Pay: £11.44-£12.20 per hour
Expected hours: 36 per week
