We are looking for an experienced Renal Nurse to join the Renal Community team. Previous experience of managing a caseload of patients is desirable, though not essential as is an in-depth knowledge of all forms of renal replacement therapies. Experience looking after patients on home therapies especially peritoneal dialysis would also be expected.

The successful applicant will have a geographical area of which they are responsible for. They will have support from the other Renal Community Nurses and wider Renal MDT to manage their patients. This role requires flexibility and the ability to work well under pressure.

The role includes managing a caseload of patients, education to patients, their families and both staff in the community and on the renal ward. Hepatitis B vaccination clinics, consultant peritoneal dialysis clinics, advanced kidney care clinics, emotional support to patients, liaising with primary care and running nurse-led local clinics. The successful candidate will also be expected to provide a support to the home haemodialysis team, therefore knowledge of home haemodialysis would be advantageous.

The successful candidate should be an excellent communicator and approachable. They should be organised and able to work both independently and part of a team.

The successful candidate will be expected to:

Assess, develop and implement specialist renal nursing care programmes and provide advice, education, support and care to people with chronic / end stage kidney disease in the community setting. Provide specialist advice to primary and secondary care teams in geographical caseload area. Provide ongoing emotional and psychological support to patients and their families. Participate in and evaluate research and clinical audit in relation to specialty to inform and improve future practice Providing education, information and advice to patients and their families regarding kidney disease and renal replacement therapy options

Working Pattern: hours per week, Monday to Friday 8: - :. On-call weekends on a rotational basis, no nights.
