Community Nursing is entering a period of exciting transformation and innovation to protect the future service we provide for our patients and sustainability of the NHS. In Line with the NHS Long Term Plan and People Plan 1, Community Nursing Services will be expanding and evolving to meet the ever-changing needs and demands of the NHS. Community Nursing provides holistic patient centred care for patients who are housebound, usually with complex, chronic and long-term conditions. Nursing for these patients required sound clinical knowledge and competence to provide sound independent judgement.

The work Community Nurses do is hugely varied so your portfolio of clinical skills can grow in complex and advanced nursing competence. Our core work includes Palliative care, supporting people at home with complex and long-term conditions, administering medications, advanced tissue viability and vascular access.

We work closely with the wider community MDT to promote admission avoidance and empowering patients to stay at home to receive their nursing care. We work very closely with our GP, Hospice, Physio and Occupational Therapy, Social Work, Voluntary and Urgent Care Services.

We currently have the opportunity to offer a substantive (permanent) contract for hours per week. We are looking for passionate, dynamic and inspired nurses to join us. For more information please read the Job Description and Person Specification to meet the needs of this role.

Our Community Nursing teams are based in Exeter to the South and West of the city. You will be based in a team of Community Nurses who a specific geographical area but at times will need to support patients across the whole city of Exeter. It is therefore vital you are an adaptive and flexible worker throughout the day. You are expected to work occasional weekends as we provide a 7-day service but this during the core business hours of the day.

You will start the day with the support of the daily virtual hub (including all the nursing teams in your cluster) to plan an effective and safe list of patient visits for the day. Then you meet later with your nursing team at patient safety handover to support each other through any concerns or advice for patient treatments or conditions as a team. Both of which are integral to our day to ensure staff work safely and within their own skill set and expertise.

The rest of your working day you could be lone working or supervising Unregistered or Student Nurses through their clinical skills, patient risk assessments and documentation. Newly appointed staff will be provided with a comprehensive induction into Community Nursing and will work alongside other Nursing staff to ensure they are supported into the post robustly.

Staff are supported with supervision both in team meetings or a one to one basis. This is also integral to our work and considered a priority to support staff at work both to develop professionally but also to support your health and wellbeing and provide a safe environment to discuss professional matters.

If you work with us there are plenty of opportunities for training and acquiring qualifications. This can either be modular based academic work, clinically focused and related to the role, leadership qualifications or the Specialist Nurse Practitioner Qualification at Master's level over two years. This qualification is a great development opportunity, is registered with the NMC and includes studying for the Independent Supplementary Prescriber qualification.

Key responsibilities of Community Nurses are:

To assess, provide and review nursing care, advice and information to patients and carers within the home environment or appropriate care setting in line with key knowledge and skills Aim to wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life. To work under the direction of the Specialist Nurse (Community) and the Nurse Team Manager (Community) using skills and knowledge as a competent Registered Nurse. To be expected to hold a designated caseload and deputise in the absence of the Specialist Nurse. Continue your professional development through additional training and education Support Unregistered team members and Student Nurses through there development by sharing clinical and leadership knowledge and skills

You must be a car driver and have use of a car. A mileage allowance is as per Agenda for Change. Currently, in all bases there is free parking however, this may be open to change.

Newly qualified staff are welcome to apply or students of nursing expecting to complete their study this year.

Nurse Associates will be considered but would be paid as per Agenda for Change for Band 4 salary. This group of staff would be excluded from the Specialist Nurse Practitioners opportunities but professional development will be encouraged.

St Thomas- Osprey House, Sowton Pinhoe- Osprey House, Sowton Topsham- Glasshouse GP practice, Topsham Ide and Exminster- Westbank Healthy Living Community Centre, Exminster Community Nursing Centre- Exeter Community Hospital, Whipton

Working Pattern: hours per week. Hours worked across a seven-day working week to include weekends.

Pay: The salary advertised for this position reflects the Agenda for Change 4-5 rates, inclusive of a pay award implemented from October 4. If you join before this date, the pay award will be backdated to April 4 or your start date, whichever is later. Until then, remuneration will be based on the 3-4 pay bandings ().
