Job summary
We are looking for an enthusiastic and creative practitioner to join our adult community learning disabilities service Intensive Support Service (ISS) as the new B7 Team Leader. The ISS is a small MDT working across Norfolk and Waveney in partnership with our colleagues at Hertfordshire Partnership NHS Trust comprising of a medic, SaLT, OT, Psychology and registered and non-registered nursing roles.
The ISS provides 7 day a week support 09:00 - 17:00 delivering care and treatment to those adults at risk of placement breakdown and admission to hospital.
The work is challenging and rewarding and you will be joining an experienced and supportive operational management and professional leadership team focused on delivering the best care and treatment we can with the recourses available.
We have given Norwich as the base for this post, however, there is flexibility as to where the successful applicant will be based, we have 5 offices across Norfolk that could be used as a primary base to suit your needs.
Main duties of the job
The post-holder will be responsible for the clinical, managerial and professional leadership of the Intensive Support Service (ISS) across the county of Norfolk. They will be responsible for the management and clinical direction of ISS clinical staff ISS to provide an intensive specialised community care for people with learning disabilities. They will also be responsible for a defined caseload of patients with complex health needs, using evidence based/patient principles to assess, plan, implement, monitor and evaluate interventions to meet the needs of the person with a learning disability and additional complex mental health/behavioural needs.

Job responsibilities
To be professionally and legally responsible and accountable for all aspects of the post holders work, including the management of people under the post holders professional care.
Using investigative and analytical skills undertake comprehensive and specialist nursing assessments as required.
To work closely alongside members of the Specialist Community Learning Disabilities Team within the wider framework of the multi-disciplinary learning disability service.
Using advanced skills in both verbal and nonverbal communication, undertake assessments with individuals whose presentation is complicated by factors associated with learning disability sensory impairment, challenging behaviours, mental health issues, and physical disability.
To ensure that core learning disability nursing skills, knowledge and experiences are maintained whilst focussing on and developing additional knowledge and skills on the core pathway deliverables.
Offer patient focussed specialist education, training, awareness of supporting patients and advice to patients, carers, other professionals and voluntary organisations.
To act as a CPA coordinator as appropriate.
Formulate individualised intervention plans, negotiating nursing goals with the individual and others as appropriate, using advanced clinical reasoning to select from a range of options those which best meet the needs of a person.
To plan and implement group work where evidence, clinical judgement and needs of the individuals indicate this is the best course of intervention.
To assess capacity, gain valid informed consent and have the ability to work within a legal framework where people lack the capacity to consent to treatment.
To monitor, evaluate and modify intervention in order to measure progress and ensure effectiveness. To supplement this with the use of an outcome measure where appropriate.
To apply a good level of understanding of the effect of disability, and provide training and advice to people themselves, family and paid carers, and multi-disciplinary team colleagues on lifestyle changes and adaptations to the clients social and physical environment.
To attend and participate in review meetings, taking formal minutes as necessary.
To promote access to mainstream services, supporting other professionals understanding of learning disabilities and reasonable adjustments.
To support individuals to complete their health books or health passports as required.
To be responsible for the equipment used in carrying out clinical duties, and to adhere to department and Trust manual handling policies, and to ensure the safe use of equipment by others.
To be able to communicate with people who do not use speech, using alternative forms of communication.
To establish strong communication networks with people who use the service, carers and other health workers and agencies.
To promote awareness of the Learning Disabilities Nursing and the ISS roles and care pathways within the specialist Community Learning Disabilities Services, the Trust, other professionals and with people with learning disabilities and their carers.
To represent the ISS at appropriate meetings as requested by NCHC Senior Managers.
To provide effective guidance, supervision and appraisal for the staff within the ISS service to manage their performance, ensure competencies are met and ensure they provide the highest quality of service.
To ensure that ISS Staff adhere to the Trusts supervision policy and provide regular clinical supervision for the staff in the ISS.
To participate in Clinical Governance, Research and Development and Audit activities as required by line manager.
To provide observation opportunities for prospective Learning Disability Nurses career candidates, students from other health professions and non-health related individuals as requested by managers.
To support colleagues to provide practice placements for Community Learning Disability Nursing students, as required.
To manage the ISS resources effectively.
To work with the Team Managers to ensure appropriate high quality local and county Specialist Community Learning Disabilities Services are being delivered in line with the services specification and in an efficient, safe and evidence-based style.
To collect, input and monitor own and other ISS staffs caseload data and report to line manager as required by the Learning Disability Service, Trust and DoH.
To inform the Head of Service of any variations to service delivery, changing priorities and needs, and their likely impact in relation to the need for service development. To work to resolve these issues with relevant others.
To monitor equipment levels in own service area and request new equipment as appropriate. To maintain a yearly stock-take of equipment in conjunction with local managers as required.
To keep assessment, treatment and information materials and equipment up-to-date, safe (following infection protocol procedures) and in good order, informing the appropriate line manager of needs and opportunities identified.
To maintain an up-to-date record of equipment loaned or prescribed to patients ensuring Trusts policies and procedures are adhered to.
To assist in the development of clinical standards, policies, procedures and further service developments.
To be aware of and adhere to service policies, procedures and systems to be used.
To participate in the operational development, planning, and implementation of policy and service development as part of the services Clinical Governance Group, leading on delegated projects as required.
To ensure that up to date (own and other ISS Staff) written and electronic records are maintained in accordance with professional and NCHC/NCC standards, and these are reviewed regularly as part of the clinical supervision sessions.
To maintain waiting lists in accordance with the services procedures and actively work to reduce these through innovative changes to service delivery and appropriate challenge.

oDegree or Diploma in Nursing (RLDN/RMN), or an Allied Health Professional or Psychology. oTo be registered and maintaining your professional registration with the relevant professional body NMC, Health and Professions Council etc.

Additional qualification within Mental Health Certificate / Diploma within Management Community Nursing Qualification.
