Job responsibilities

The following are thecore responsibilities of the Advanced Clinical Practitioner.

a.Thedelivery of highly effective medical care to the entitled population

b.Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice to patients.

c.Implementand evaluate individual specialised treatment plans for chronic diseasepatients

d.Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects on the patients health.

e.Provideadvanced, specialist nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF

f.Requestpathology services as necessary

g.Processand interpret pathology and other test results as required
