Main duties of the job

The successful applicant will support patients in preparingfor or in following-up clinical conversations they have with primary careprofessionals. The applicant will work closely with the GPs and other primary careprofessionals within the PCN to identify and manage a caseload of identifiedpatients, making sure that appropriate support is made available to them andtheir carers and ensuring that their changing needs are addressed.

As one of the largest teaching GP Practices in Bristol,Downend Health Group (DHG) cares for over 27,000 patients and this numbercontinues to rise each year.

This excellent facility offers patients a family friendlyservice with efficient and easy access to a highly trained team of health careprofessionals. The medical team comprises 12 Partners (10 GPs, and 2Pharmacists), over 18 Salaried GPs, Clinical Pharmacists, Paramedics and adedicated team of Nurses and Healthcare Assistants. We are supported by ourexperienced Care Co-ordinator Team, Medicines Management Team, AdministrationTeam and Reception Team who provide excellent support both front of house andbehind the scenes.

The surgery provides modern healthcare services and bothbuildings offer calm, welcoming environment for patients, with enhanced andpleasant working conditions for clinicians and staff.

We use EMIS Web and electronic prescribing. We also useDocman scanning software to allow efficient scanning and letter managementacross both sites.

Carecoordinators will:

Proactivelyidentify and work with a cohort of patients to support their personalised carerequirements, using the available decision support aids. These include patients with a LearningDisability, Mental Health, Dementia and vulnerable/frail patients. Bringtogether all of a persons identified care and support needs and explore theiroptions to meet these into a single personalised care and support plan (PCSP)based on what matters to the person, ensuring dignity, choice, respect,independence and rights are upheld at all times. Helppeople to manage their needs, answering their queries and supporting them tomake appointments, and ensuring that people have good quality written or verbalinformation to help them make choices about their care. Identifybarriers to or issues preventing greater engagement and/or uptake by patientsactively participate in developing solutions for patients and serviceproviders. Supportpeople to take up training and employment, and to access appropriate benefitswhere eligible. Raiseawareness of shared decision making and decision support tools and assistpeople to be more prepared to have a shared decision-making conversation. Ensurethat people have good quality information to help them make choices about theircare, support people to understand their level of knowledge, skills andconfidence (their Activation level) when engaging with their health andwellbeing, including through use of the Patient Activation Measure. Assistpeople to access self-management education courses, peer support orinterventions that support them in their health and wellbeing. Exploreand assist people to access personal health budgets where appropriate. Providecoordination and navigation for people and their carers across health and careservices, alongside working closely with social prescribing link workers,health and wellbeing coaches and other primary care professionals. Supportthe coordination and delivery of MDTs within practice. MonitorDNA patients in Vulnerable patient groups. Workingwith GPs to provide ongoing support to parents who may be struggling with theirchilds behaviour, sleep, anxiety and mental health or awaiting a childassessment for ADHD/ASD. Signposting these parents to support groups andcharities. Coordinating referrals whennecessary and submitting Right to Choose ASD and ADHD assessment requests.Liaising with other professionals to support families, Social Care, Healthvisitors Anyother duties as requested by your line manager.
