We are looking for a skilled and highly motivated Practice Lead to jointhe Exeter Community Mental Health teams.
The service is embarked on a challenging but exciting journey with theimplementation of the Community Mental Health Framework. The successfulapplicant will be an innovative and compassionate clinical leader, who ismotivated to support the teams to deliver individualised, person-centred careand will make a significant contribution to service development of the ExeterCity Community Team. You will join a rich multi-disciplinary team which includes Nurses,Occupational Therapists, Social Workers, Psychologists, Psychiatrists, PeerSupport Workers, Clinical Associate Psychologists and Community SupportWorkers. We are also forming closer working relationships with colleagues inPrimary Care and the Voluntary Sector.

You will be an experienced mental health professional (Registered Nurse,Social Worker or Occupational Therapist) who has a genuine interest incollaborative working, patient safety and delivering high quality care.
Main duties of the job
TheCommunity Practice Lead is responsible for ensuring compliance with quality& safety standards within their assigned clinical pathways / teams. Theywill ensure that all individuals receiving care and treatment from DPT servicesdo so in line with Trust strategic objectives (safe, timely, personalised,sustainable and recovery focused) and are safeguarded from any harm.
Workforceplanning, audit, compliance with CQC standards, serious untoward incidents,complaints and compliments and associated learning, and clinical practiceeducation are the key responsibilities of this role. The post holder willensure clinicians work within DPT policy and practice standards, and that theyadhere to professional registration requirements.
Thepost holder will be an effective role model for clinical excellence acrosstheir assigned clinical pathway. Theywill demonstrate visible leadership and take responsibility for ensuring atherapeutic environment.

Service design &development
Thepost holder will be responsible for the development and monitoring of theassigned care pathway quality and improvement plans. The post holder, inconjunction with the CMHT management team, will ensure all clinicaleffectiveness systems are embedded and sustainable -this will include audit andevidence based care.
Participatein, and lead as required, the development, implementation and evaluation ofstrategies and clinical policies to ensure the service continues to evolve tomeet the needs of the individuals and / or their carers.
Service delivery &resource management
Ensurethat individuals care pathways are monitored and that individuals transition throughthe service appropriately.
Ensurethat governance and assurance systems are in place for care pathways / clinicalteams and provide expert professional advice with regard to clinical care,incidents and complaints.
Ensurethat services are delivered by practitioners with the appropriate knowledge,skills and competencies.
Inconjunction with the Community Team Managers, ensure that care is providedwithin the CMHT resources.
Practice quality &workforce development
Takethe lead, in consultation with the CMHT management team, in being responsiblefor:-
Theimplementation of the Care Programme Approach within all Trust services is inaccordance with the recovery co-ordination policy.
Theco-ordination of multi-disciplinary team working and engagement in care pathwaydelivery in line with practice standards.
Ensuringthe safety and effectiveness of the service through the implementation ofeffective risk management, governance and assurance systems.
Developthe LDU workforce development plan.
Line ManagementResponsibility
Beresponsible for ensuring the clinical supervision and Continuous Professional Developmentof pathway staff.
Provideappropriate cross-cover for supervisions and PDRs as required.
AdditionalResponsibilities
Allpractice leads will hold a portfolio of LDU responsibilities these willinclude: ensuring the quality and triangulation of provider complianceassessment tools and responsibility for updating and monitoring risk registers.
Theywill also take responsibility for key practice development projects across thedirectorate as assigned by the Head of Practice and Professions.
All Community Practice Leaders will hold an LDUportfolio based on individual job planning safeguarding, complaints.

An excellent understanding of the NSF and Mental Health Act/ Mental Health Capacity Act requirements. CPA Process Understanding of recovery principles in Mental Health Knowledge of infection control Working knowledge of safeguarding processes (adult and child) Knowledge and understanding of the Community Mental Health Framework

Extensive experience of managing a busy caseload of clients with a range of mental health problems. Substantial experience of working at a senior clinical level. Working within a community mental health setting in either the statutory or voluntary sector. Providing clinical supervision and managing staff.
