About the role:

Accept referrals, assess need and make appropriate recommendations for care. Consult with Primary Care (GP’s), community mental health services, mental health hospitals, IAPT services and other providers of mental health and substance use support to give services users a robust package of care to support their recovery. Be responsible for maintaining a full case load of service users with complex needs. Conduct comprehensive assessments, construct care packages with multi-disciplinary partners where required, review care plans in a structured format and manage onward referrals. Conduct home visits to service users to support those with disabilities and to those with family responsibilities. Develop and sustain links with multi-disciplinary including statutory and non-statutory providers, Criminal Justice and wrap around services.

About you:

Considerable experience of working with people with Substance Use. Experience of working Co-occurring conditions. Sound knowledge of drugs and alcohol and their effects Knowledge of blood borne viruses and sexual health Excellent communication skills Case management experience Experience of using techniques such as CBT and Motivational Interviewing
