Job summary
We are looking for a Clinical Pharmacist for 24 hours per week to join our established Pharmacy Team. The successful candidate will become part of a friendly and supportive team which includes 2 part time pharmacists and a shared (with the PCN) Pharmacy Technician. You will be involved in regular meetings both within the pharmacy team and with the whole practice team. The practice belongs to Hillsborough PCN, which has a very collaborative Pharmacy Team.
For an applicant who prefers fulltime it may be possible to share the post with another of the PCN practices
Main duties of the job
You will provide support to general practice staff with regards to prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests and undertake medicines reconciliation on transfer of care. Provide clinical leadership on medicines optimisation and quality improvement and be involved in the management of some aspects of the quality and outcomes framework and enhanced services
About us
Dykes Hall Medical Centre is a two branch practice with approx. 9,000 patients. Our main site is in Hillsborough and the branch site is in Stannington.
The practice team consists of 4 partners, 3 salaried GP's, 2 Advanced Nurse Practitioners, 1 Lead nurse and 2 Practice nurses, 3 Health Care assistant. We employ a Physician's Associate and 2 well established pharmacists.
The practice is a training practice and we usually have 2 F2 Doctors and 4 Trainees.

Job Description
Support to patients regarding acute and repeatprescription queries.
This will be a mixture of telephone and virtualwork for patients with questions and concerns about their medicines.
Providing follow up for patients to monitor theeffect of any changes.
Medication information to practice staff
Answering relevant medicine-related enquiries fromGPs, other practice staff, other healthcare teams ( community pharmacy)
Suggesting and recommending solutions.

Management of medicines at discharge from hospital
To reconcile medicines following discharge fromhospitals or intermediate care, including identifying and rectifyingunexplained changes and working with patients and community pharmacists toensure patients receive the medicines they need post discharge.
Set up and manage systems to ensure continuity ofmedicines supply to high risk groups of patients ( those with medicinecompliance aids or those in care homes).
Signposting
Ensure that patients are referred to theappropriate healthcare professional for the appropriate level of care within anappropriate period of time pathology results, common/minor ailments, acuteconditions, long term condition reviews etc.
Repeat prescribing
Manage the repeat prescribing reauthorisationprocess by reviewing patient requests for repeat prescriptions and reviewingmedicines reaching review dates and flagging up those needing a review.
Ensure patients have appropriate monitoring testsin place when required.
Medicines quality improvement
Undertake clinical audits of prescribing in areasdirected by the GPs, feedback the results and implement changes in conjunctionwith the practice team.
Medicines safety
Implement changes to medicines that result fromMHRA alerts, product withdrawal and other local and national guidance.
Care Quality Commission
Work with the general practice team to ensure thepractice is compliant with CQC standards where medicines are involved.
