Job Summary
We are seeking a highly skilled Clinical Pharmacist to join our team at Green Cross Group Practice. As a Clinical Pharmacist, you will play a vital role in supporting the delivery of high-quality patient care and contributing to the development of our Primary Care Network.
Main Responsibilities
- Support the team with medication and prescription queries, conducting structured medication reviews, and assisting in quality improvement initiatives to ensure cost-effective, safe, and environmentally sustainable medication management.
- Manage medicines reconciliation during care transitions, supporting the repeat prescription system, and handling acute prescription requests.
- Contribute to systems for safer prescribing and provide expert clinical medicines advice, addressing both public and social care needs of patients within the GP practices.

Key Requirements
- Completion of the Pharmacist Foundation programme or a minimum of 2 years experience as a pharmacist within the NHS or community Pharmacy.
- Registration with the General Pharmaceutical Council.
- Desirable: experience working in community pharmacy, general practice, or primary care setting, and completion of independent prescribing qualification or Primary Care Pharmacy Education Pathway.
