Practice Nurse

We are seeking an experienced and skilled Practice Nurse to join our team at Primary Care Sheffield. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the delivery of primary care services.
Main Responsibilities:- Provide comprehensive patient care, including screening, clinical assessment, diagnosis, treatment, and follow-up care.
- Manage chronic diseases, such as asthma, COPD, and diabetes, and provide health promotion and education to patients.
- Undertake routine tasks, such as cervical cytology, vaccinations, and phlebotomy.
- Work collaboratively with the clinical team, including GPs and other healthcare professionals, to ensure delivery of a coordinated service.
- Participate in quality assurance strategies, such as audit and peer review, to monitor the effectiveness of clinical practice.
- Minimum of 2 years post-registration experience in primary care.
- Experience in managing long-term conditions, health promotion, and education.
- Knowledge of national standards, policy developments, and evidence-based practice.
- Excellent interpersonal, verbal, and written communication skills.
- Ability to work autonomously and as part of a team.
