Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Foxhill Medical Centre. As a Clinical Pharmacist, you will play a vital role in improving patient health outcomes and the efficiency of our primary care team.

Key Responsibilities:
  • Provide direct, accessible, and timely medicines and expertise to patients, including face-to-face contact.
  • Work closely with the primary care team to optimise medication issues and improve patient care and safety.
  • Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews.
  • Provide specialist expertise in the use of medicines and help address public health and social care needs.
  • Lead on person-centered medicines optimisation and quality improvement initiatives.
  • Support patients to take their medications effectively, reduce waste, and promote self-care.
  • Develop relationships with other pharmacy professionals across the wider health and social care system.
  • Take a central role in clinical aspects of shared care protocols, clinical research, and liaison with specialist pharmacists.
Secondary Responsibilities:
  • Support delivery of quality and cost-effectiveness initiatives.
  • Improve data quality of medicines records and linking to conditions.
  • Work with hospital pharmacists to improve prescribing safety after discharge.
  • Provide support to prescription administrative/dispensary staff.
  • Produce pharmacy updates and work with community pharmacists to improve patient experience.
  • Support virtual and remote models of consultation and support.
