About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Green Cross Group Practice. As a Clinical Pharmacist, you will play a vital role in supporting the delivery of high-quality patient care and contributing to the development of our pharmacy services.

Key Responsibilities

As a Clinical Pharmacist, you will be responsible for:

  • Conducting structured medication reviews with patients and making adjustments to their medication regimens as needed.
  • Managing caseloads of patients with common/minor/self-limiting ailments, working within a scope of practice and limits of competence.
  • Providing expert clinical medicines advice to patients, GPs, and other healthcare teams.
  • Identifying and addressing medication-related risks and issues through risk stratification and quality improvement initiatives.
  • Contributing to the development and implementation of new services that have medicinal components, such as treatment pathways and patient information leaflets.

Requirements

To be successful in this role, you will need:

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • A minimum of 2 years' experience as a pharmacist within the NHS or community pharmacy.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, GPs, and other healthcare teams.
