Job Summary

We are seeking a skilled Clinical Pharmacist to join our team at Green Cross Group Practice. As a Clinical Pharmacist, you will play a vital role in supporting our patients with medication-related queries, conducting structured medication reviews, and contributing to quality improvement initiatives.

Main Responsibilities

The successful candidate will work closely with our practice team to:

  • Support patients with medication-related queries and provide expert clinical advice
  • Conduct structured medication reviews to optimize patient care
  • Assist in quality improvement initiatives to ensure safe and effective medication management
  • Manage medicines reconciliation during care transitions
  • Contribute to systems for safer prescribing and provide expert clinical medicines advice

Requirements

To be successful in this role, you will need:

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • A minimum of 2 years' experience as a pharmacist within the NHS or community pharmacy
  • Excellent communication and interpersonal skills
  • A strong commitment to patient-centered care
