Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Green Cross Group Practice. As a Clinical Pharmacist, you will play a vital role in supporting the delivery of high-quality patient care and contributing to the development of our pharmacy services.

Key Responsibilities:
  • Conduct structured medication reviews with patients and make adjustments to long and short-term medication as needed.
  • Manage caseloads of patients with common/minor/self-limiting ailments, working within a scope of practice and limits of competence.
  • Provide expert clinical medicines advice to patients, GPs, and other healthcare teams, addressing public and social care needs.
  • Support the repeat prescription system, handle acute prescription requests, and manage medicines reconciliation during care transitions.
  • Contribute to systems for safer prescribing and participate in quality improvement initiatives to ensure cost-effective, safe, and environmentally sustainable medication management.
Requirements:
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Minimum of 2 years' experience as a pharmacist within the NHS or community pharmacy.
  • Experience working in a general practice or primary care setting is desirable.
