Job Title: PCN Clinical Pharmacist Prescriber

Job Summary:

The Clinical Pharmacist will work closely with the PCN Pharmacy Team and will provide clinical support to the pharmacy technicians and clinicians. The pharmacist will support the PCN and practice clinical team with queries around medication, acute prescription requests and medicines reconciliation following discharge from another care provider, medication reviews, supporting self-care, minimizing clinical risk, answering complex medicines queries, and aiming to reduce wasted medicines.

Main Responsibilities:

  • To work as part of a multi-disciplinary team whilst in a supporting function to the Pharmacy Team and other clinicians and within agreed procedures.
  • Undertake medication reviews to support improved patient outcomes including increased medication adherence, stopping medications that are causing harm, and optimizing medication regimens.
  • Participate in pharmacy meetings, and other meetings to improve engagement and to promote issues relevant to prescribing and medicines optimization.
  • Support the Medicines Optimization team in the delivery of the Medicines Optimization strategy in Sheffield.
  • Handle function-specific information, which may be sensitive, complex or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldecott Guidelines and the Confidentiality Code of Conduct.
  • Be responsible for the organization, planning and management of own workload to meet set deadlines.
  • Utilize GP practice clinical systems to conduct prescribing audits SystmOne/Emis.
  • Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate timeframe and to advise on any medication changes that occur as a result.
  • Undertake training and development, to ensure the required knowledge and skills for practice-based work.
  • Carry out other duties which are appropriate to the skills and competencies of the post holder and grade.
  • Establish and maintain effective working relationships with GP practices, CCG and other healthcare professionals/stakeholders across the PCN.
  • Provide direct support and advice to GP practices and health professionals to promote and implement high-quality prescribing to the benefit of patients.
  • Provide peer support to other clinical pharmacists in the PCN.
