Position Overview

The Primary Care Clinical Pharmacist will collaborate closely with the Pharmacy Team within the Primary Care Network (PCN) to deliver essential clinical support to pharmacy technicians and healthcare professionals. This role involves addressing inquiries related to medications, managing urgent prescription requests, and conducting medicines reconciliation following patient transitions from other healthcare settings. The pharmacist will also engage in medication reviews, promote self-care initiatives, minimize clinical risks, respond to complex medication inquiries, and strive to reduce medication waste. It is essential that the candidate holds prescriber qualifications.

The ideal candidate must possess prescriber qualifications, and prior experience in primary care is advantageous.

About Our Team

Our dedicated team consists of four clinical pharmacists and four pharmacy technicians serving a diverse patient population.

We cater to a community of 36,000 patients across multiple practices.

Core Duties

The following outlines the primary responsibilities of the Primary Care Clinical Pharmacist. Additional tasks may be assigned based on workload and staffing needs:

  • Function as an integral member of a multidisciplinary team, providing support to the Pharmacy Team and other clinicians in accordance with established protocols.
  • Conduct thorough medication reviews aimed at enhancing patient outcomes, including:
    • Improving medication adherence
    • Discontinuing medications that may be detrimental
  • Participate in pharmacy meetings and other collaborative discussions to foster engagement and address issues pertinent to prescribing and medication optimization.
  • Assist the Medicines Optimisation team in executing the Medicines Optimisation strategy.
  • Manage sensitive and complex information in compliance with relevant data protection regulations, ensuring proper documentation and coordination.
  • Organize and prioritize personal workload to meet established deadlines.
  • Utilize clinical systems for conducting prescribing audits.
  • Act on significant drug alerts as appropriate and advise on necessary medication adjustments.
  • Engage in ongoing training and professional development to maintain required competencies.
  • Perform additional duties aligned with the skills and qualifications of the role.
  • Establish and nurture effective working relationships with GP practices and other healthcare stakeholders within the PCN.
  • Provide direct support and guidance to GP practices and health professionals to enhance high-quality prescribing practices for patient benefit.
  • Offer peer support to fellow clinical pharmacists within the PCN.
