Position Overview

The Primary Care Clinical Pharmacist will collaborate closely with the Pharmacy Team within the Primary Care Network (PCN) to deliver essential clinical support to pharmacy technicians and healthcare professionals. This role is pivotal in addressing medication-related inquiries, managing urgent prescription requests, and performing medication reconciliation following patient transfers from other healthcare facilities. The pharmacist will also conduct medication reviews, promote self-care initiatives, minimize clinical risks, and provide solutions to complex medication queries, all while striving to reduce medication waste. It is essential that the candidate holds prescriber qualifications.

About Our Organization

Our dedicated team consists of four clinical pharmacists and four pharmacy technicians, serving a diverse patient population.

Core Duties

The following responsibilities are integral to the role of the Primary Care Clinical Pharmacist. Additional tasks may be assigned based on workload and staffing needs:

  • Engage as a member of a multidisciplinary team, providing support to the Pharmacy Team and other clinicians within established protocols.
  • Conduct comprehensive medication reviews aimed at enhancing patient outcomes, including:
    • Improving medication adherence.
    • Discontinuing medications that may cause adverse effects.
  • Participate in pharmacy meetings and other collaborative sessions to foster engagement and address issues pertinent to prescribing and medication optimization.
  • Assist the Medicines Optimisation team in executing the Medicines Optimisation strategy.
  • Handle sensitive and complex information in accordance with relevant data protection regulations, ensuring confidentiality and proper documentation.
  • Organize and manage personal workload to meet established deadlines.
  • Utilize clinical systems for conducting prescribing audits.
  • Respond to major drug alerts promptly and advise on necessary medication adjustments.
  • Engage in ongoing training and professional development to maintain requisite knowledge and skills.
  • Perform additional duties aligned with the skills and competencies of the role.
  • Establish and nurture effective working relationships with GP practices and other healthcare stakeholders.
  • Provide direct support and guidance to GP practices to promote high-quality prescribing practices.
  • Offer peer support to fellow clinical pharmacists within the PCN.
