Job Summary

We are seeking a highly motivated and experienced Clinical Pharmacist to join our Pharmacy Admissions and Discharge Team at Sheffield Teaching Hospitals NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality clinical care to patients across various departments.

Main Responsibilities
  • Provide specialist clinical pharmacy services to medical and surgical admission patients, supporting the Principal Pharmacist for PADT.
  • Attend Post Take Ward Rounds for acute medicine on a rota basis, contributing to the development of clinical skills and knowledge.
  • Perform independent pharmacist prescriber role, utilizing your expertise to optimize patient care.
  • Develop and implement guidelines, protocols, and SOPs with the support of a senior pharmacist, ensuring seamless service delivery.
  • Contribute to service development projects, working collaboratively with the Principal Pharmacist and other stakeholders.
  • Provide support and mentorship to post-registration pharmacists, promoting their growth and development.
  • Participate in the education and training of pre-registration pharmacists, other pharmacy staff, and healthcare professionals, fostering a culture of excellence.
  • Support pharmacy service areas, including dispensaries, technical, and medicines information, ensuring a comprehensive service offering.
  • Contribute to the out-of-hours service, including evenings and weekends, maintaining a high level of service delivery.

Requirements
  • Registered Pharmacist with the General Pharmaceutical Council.
  • MPharm in pharmacy.
  • Post-graduate Certificate in Clinical Pharmacy (60 credits passed).
Desirable Qualifications and Experience
  • Post-graduate Diploma in Clinical Pharmacy.
  • Independent Prescriber.
  • Member of the Royal Pharmaceutical Society of Great Britain.
  • Significant post-registration experience in clinical pharmacy.
  • Experience in a wide range of clinical pharmacy specialties, including acute medical admissions.
  • Experience of working in a multidisciplinary team and in UK hospital pharmacy.
Skills and Knowledge
