This exciting post serves the Health Board, Local Authority and third sector partners as part of a small team of transformation professionals, to drive the integration of health and social care services in Pembrokeshire.

Working closely with a range of stakeholders, you will be instrumental in building a rich project portfolio, a robust integrated plan and a quality assured programme of delivery which focuses on making a difference to the health and wellbeing outcomes of our population.

Main duties of the job

You will turn the vision of leaders into front line services and establish the organisational relationships necessary to make this happen, easing tensions, encouraging joint working and bringing solutions to the table.

The job involves a stimulating analytic component, developing measures of cost, performance and user/patient experience, alongside the management of a bundle of transformation projects. Above all, you will be a problem-solver and bridge-builder, simplifying where others see complexity and bringing clarity when misunderstanding threatens to derail progress.

You will also be the senior member of the small team reporting to the Head of Integrated Transformation and, as such, will coordinate the programme and guide other team members in addition to delivering your own projects. You are likely to be familiar with delivering complex projects and resisting scepticism and have a desire to be a part of making change happen.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:

Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales

Qualifications and Knowledge

Essential

Masters level study or equivalent experience Evidence of continuous project management development including recent post graduate study Recognised project management qualification or equivalent demonstrable experience Must have knowledge and understanding of current policy and issues in professional practice, compliance, standards across health and social care Must possess a thorough understanding of the role and its purpose within the organisation Strong IT skills including the use of spreadsheets to a high standard

Desirable

Clear understanding of the transformation agenda, opportunities for redesigning services and the tools / techniques for achieving service change Mentoring / Coaching qualification Demonstrable knowledge of quality improvement methodologies Understanding of performance measurement metrics and results-based evidence approaches Understanding of project quality assurance methodology

Experience

Essential

Demonstrable significant experience leading and managing projects in a public sector or comparable organisation Significant experience in a managerial role in a comparable environment Experience of budget setting and monitoring financial position Experience of providing and receiving highly complex, sensitive and contentious information Experience of developing and implementing project plans Experience of working in partnership with other organisations Must have direct experience of major service and organisation change and knowledge of how to navigate a service through a successful change programme
