An exciting opportunity has arisen for a part-time Care Co-ordinator to cover GP Surgeries across North Pembrokeshire.
We have an ambitious vision to deliver integrated care to patients across the North Pembrokeshire patient population of Haverfordwest and Milford Haven.
We are looking for a dedicated, enthusiastic, and proactive professional. Someone who is highly motivated, with a caring and empathetic approach, who will work as the Co-ordinator for the Multi-Disciplinary Teams (MDT) of two of our GP surgeries.
By supporting members of the MDT meetings, you will be responsible for ensuring the most appropriate service and health professional is treating patients at the optimum time. Ensuring a consistency of approach, and avoiding duplication of visits, this care will be largely delivered to patients in their own homes.
To be the first point of contact for health care professionals across the county, predominately in General Practice, and to act upon patient referral information at a time of escalated needs and co-ordinate timely and holistic response of the professional team, ensuring no duplication of service provision.
This role supports the integration of Primary Care with the the integrated care team within the County to deliver effective, co-ordinated care for vulnerable and frail adults, including those requesting frequent GP contacts and visits, and those at those at high risk of a hospital emergency admission, ED attendances or out of hours care.
To improve lines of communication between the community hubs (Integrated Care Team, District Nurses) and General Practice and support the continued development of joint Community and Primary Care models of multi-disciplinary team working. This will include liaising with Community Connectors, Primary Mental Health teams and Third Sector providers.
To support GPs, Primary Care professionals and their patients by arranging for the right service and professional to be with them, at the right time, helping to ensure a consistency of approach; specifically, although not exclusively, around patients in their own homes.
Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (five of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
