Job summary

We are looking for a motivated and enthusiastic Advanced clinical practitioner to join our friendly, patient focused practice team.

- Assigned GP for supervision/support

- Planned coffee break during the day with the rest of the team

- Don't need to file bloods/letters

- Minimal home visits

- Teaching atmosphere with a strong emphasis on this

Previous experience in minor illness and triage ina primary care health experience is essential. You must be able to demonstratesafe, clinical decision making for patients acting within professionalboundaries.

Applicants will have completed their NursePractitioner qualification or equivalent and be confident working in primarycare, with the support of our GP team.

The post holderwill be an experienced nurse practitioner who, acting within their professionalboundaries, will provide care for the presenting patient from initial historytaking, clinical assessment, diagnosis, treatment and evaluation of care. Theywill demonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills, for patients within the general practice.

They willdemonstrate critical thinking in the clinical decision-making process. Theywill work collaboratively with the general practice team to meet the needs ofpatients, supporting the delivery of policy and procedures, and provide leadershipas required.

In order to workat this level NMC requirements or equivalent for advanced practice must be met.

You will be joining a team of 5 GP partners, 3 Salaried GPs, NursePractitioners, a team of Practice Nurses and HCAs, who are supported by Management, Admin and Reception staff.

Job responsibilities

Clinically examine and assesspatient needs from a physiological and psychological perspective, and planclinical care accordingly by telephone triage or face to face consultations.

Assess, diagnose, plan,implement and evaluate interventions/treatments for patients with complexneeds. Proactively identify, diagnose and manage treatment plans for patientsat risk of developing a long-term condition (as appropriate)

Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-based treatmentmethods into a management plan

Prescribe and reviewmedication for therapeutic effectiveness, appropriate to patient needs and inaccordance with evidence-based practice and national and practice protocols,and within scope of practice

Work with patients in order tosupport compliance with and adherence to prescribed treatments. Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects and interactions

Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care

Support patients to adopthealth promotion strategies that promote healthy lifestyles, and applyprinciples of self-care

Utilise and demonstratesensitive communication styles, to ensure patients are fully informed andconsent to treatment

Deliver a quality service

Recognise and work within owncompetence and professional code of conduct as regulated by the NMC. Produceaccurate, contemporaneous and complete records of patient consultation,consistent with

Adhere to legislation, policies andprocedures and participate in audit.

Prioritise, organise andmanage own workload in a manner that maintains and promotes quality

Assess effectiveness of caredelivery through self and peer review, benchmarking and formal evaluation
