**About the Role**:
Clacton PCN is looking to recruit an advanced clinical practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries.

Clacton Primary Care Network (PCN) comprises of 6 local GP practices:

- Eastlynne Medical Centre (Employer and lead practice)
- Old Road Surgery
- North Clacton Medical group
- Thorpe surgery
- Fronks Road
- Harewood surgery

This role will be based at one or more of the PCN practices dependant on needs and demand at time of recruitment.

The post-holder will:

- demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families and carers.
- be practising autonomously as an advanced clinical practitioner within the designated speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate.
- support new ways of working that emphasises a more efficient and patient-focused service, and will ensure the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area.
- have responsibility for leading the on-going development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues.

Qualifications, skills and experience required:

- Professional registration with relevant professional body: NMC, HCPC or GPhC.
- Post-graduate qualification in specific speciality, or equivalent experience.
- Either graduated from a centre for Advancing Practice accredited MSc advanced practice or completed the Centre’s ePortfolio (supported) route.
- Post-registration experience at a senior level (Band 7 or above) in specialty area.
- Evidence of on-going CPD.
- Advanced clinical assessment skills.
- Excellent communication skills.
- Ability to perform in stressful and complex situations.
- Sound clinical reasoning and decision making skills.
- Flexibility.
- Ability to inspire and lead a team.

Our primary care network has various clinical and non
- clinical roles which consist of the following: paramedics, clinical pharmacists, physiotherapists, cancer care nurse, care coordinators, GP assistants, adult mental health clinicians, physician associates, children’s mental health worker and children’s councillor as well as social prescribers. All of these roles work towards bridging the gap between primary and secondary care as well as supporting our member practices to improve the assessment and service that we collectively provide.
