Essential Qualifications
Prior experience in managing minor illnesses and triage within a primary care setting is crucial. You must demonstrate safe clinical decision-making while adhering to professional boundaries.
Applicants should possess a Nurse Practitioner qualification or its equivalent and be confident in delivering primary care with the support of our GP team.
Key Responsibilities
The successful candidate will be an experienced nurse practitioner who, within their professional scope, will:
- Provide comprehensive care for patients, including initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
- Exhibit critical thinking in clinical decision-making processes.
- Collaborate with the healthcare team to address patient needs, ensuring adherence to policies and procedures.
- Provide leadership as necessary.
To qualify for this role, candidates must meet the NMC requirements or equivalent for advanced practice.
Main Duties
As a Senior Clinical Practitioner, you will be expected to:
- Conduct clinical examinations and assess patient needs from both physiological and psychological perspectives.
- Plan and implement clinical care through telephone triage or face-to-face consultations.
- Diagnose and manage both acute and chronic conditions, integrating various treatment methods into a comprehensive management plan.
- Prescribe and review medications, ensuring therapeutic effectiveness in line with evidence-based practices.
- Support patients in adhering to prescribed treatments and provide information on medication regimens.
- Prioritize health issues and intervene appropriately in complex or urgent situations.
- Encourage health promotion strategies that foster healthy lifestyles and self-care principles.
- Utilize effective communication styles to ensure patients are well-informed and consent to treatment.
- Maintain accurate and complete records of patient consultations.
- Participate in audits and adhere to relevant legislation, policies, and procedures.
- Manage your workload effectively to promote quality care delivery.
- Engage in quality governance systems and processes across the practice.

- At least two years of recent experience in primary or community care.
- Relevant diplomas in managing long-term conditions.
