Job SummaryWe are seeking a highly skilled and experienced Clinical Lead Nurse Practitioner to join our team at Woodlands Family Medical Centre. The successful candidate will be responsible for providing high-quality patient care, leading clinical teams, and contributing to the development of our healthcare services.
Key Responsibilities- Provide autonomous clinical decision-making and expert care for patients within the general practice.
- Lead clinical teams and contribute to the development of our healthcare services.
- Develop and implement patient-centered strategies to address community health needs.
- Collaborate with multidisciplinary teams to meet the needs of patients and support the delivery of policy and procedures.
- Contribute to the evaluation and audit of clinical practice and contribute to the development of computer-based patient records.
- Participate in training and professional development to maintain expertise and stay up-to-date with best practices.
- Recognized NP qualification at minimum of degree level or equivalent Independent Nurse Prescriber.
- Previous post-registration experience in a primary care setting.
- Experience in managing long-term conditions, such as asthma, COPD, diabetes, and CHS.
- Proven ability to evaluate the safety and effectiveness of their own clinical practice.
- Understanding and knowledge of policy developments related to the delivery of primary care services, including general practice, clinical governance, and quality and outcomes framework.
- Full and valid driving license and access to a car for work purposes.
- Current registration and appropriate representation with a recognized medical defense organization.
- Experience and interest in a specific disease area.
- Teaching/mentoring experience and/or qualification.
- Interpreting and implementing local and national policy.
- Strong levels of IT proficiency and familiarity with primary care clinical systems.
- Experience in using the Microsoft suite of programs.
