Job summary

The post holder will be responsible for the assessment, co-ordination and the delivery of a high quality mental health service for patients under the care of CATT who required assessment by secondary mental health services for pathway planning, brief interventions (up to 12 weeks), crisis intervention in the community, or gate keeping for the inpatient wards and community recovery house with the emphasis for potential providing home treatment.

The post holder will be involved in the on-going development of services, to ensure that the most relevant care pathway and care plan is devised to provide access to urgent and crisis care. They will be active in the promotion and use of evidence-based practice through the provision of clinical advice and support; acting as a role model; and providing education and training where appropriate. They will liaise closely with carers, both statutory and non-statutory services, emergency services, and 3rd sector services. They will liaise closely with local services Single Point of Access providing rapid response to mental health crisis situations where necessary and appropriate to decrease the number of unnecessary presentations to A&E. The post holder will also deliver supervision for junior colleagues. To work within a 24 hour 7 day a week rota system, ensuring that a comprehensive service is provided to patients and robust gatekeeping is maintained.

Main duties of the job

To discuss the assessment to be undertaken with the work team and understand his/her own role in the overall assessment and the activities to be undertaken.

To respect individuals' dignity, wishes and beliefs and obtain their consent for the assessment to be undertaken.

To identify appropriate methods, techniques and equipment for different activities taking into account any risks.

To undertake and record assessment activities as agreed with the care team, following established protocols/procedures and consistent with legislation, policies and procedures.

To report assessment findings in the appropriate format to the people who need them.

To offer to the team his/her own insights into the health and well-being needs and wishes of the service users and make suggestions on the treatment that might be needed.

To maintain awareness of Trust medicine management policy and protocols and ensure adhered to practice guidance, as they are updated and respond to national and local guidance.

Qualifications

Essential

Registered Professional (Mental Health Nurse, Social Worker or OT). Relevant post registration experience. Evidence of post registration further development.

Desirable

Degree in Health Care related subject. Qualification in psychosocial interventions in mental health. KUF training

Experience

Essential

Post qualification experience to include appropriate experience within an acute mental health care setting and / or significant experience within community, crisis care, or liaison psychiatry. Experience of working with other professions within a multi-disciplinary team Experience of working independently

Desirable

Advanced Assessment Skills training. Experience with telephone triage within a Single Point of Access Service. Experience of Psychiatric Liaison services.

Knowledge

Essential

A high level of both written and spoken English language. Knowledge of relevant mental health law. Knowledge of adult and children safeguarding requirements.

Desirable

Excellent communication skills including common courtesy and listening skills. Accredited training in customer service. Knowledge of RiO patient record system.
