Community Psychiatric Nurse

We are seeking a skilled Community Psychiatric Nurse to join our team of mental health professionals. This role offers a unique blend of professional fulfillment and adaptable scheduling, providing vital healthcare services within the community.
The successful candidate will have a minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years. They must also hold NMC Registration and comply with current nursing practices. A full UK driving licence and own vehicle are essential requirements.
Job Responsibilities- Providing robust assessment of patients (telephone or face-to-face) referred due to mental health difficulties
- Identifying appropriate care pathways and ensuring the least restrictive option to meet patient needs and ensure their well-being
- Signposting to appropriate care providers (primary and tertiary care)
- Completing documentation and planning care for patients
- Presenting cases to Multi-Disciplinary Team for discussion around care pathway
- A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years
- NMC Registration statement
- Degree / Diploma in Nursing
- Full UK driving licence and own vehicle
